By submitting my written and/or video testimonial, I hereby give ACS Technologies permission to use quotes from the interview(s) (or excerpts of such quotes), the film, photograph(s), tape(s)or reproduction(s) of me, and/or recording of my voice, in part or in whole, in publications, newspapers, magazines and other print media, on television, radio and electronic media (including the Internet), in promotional mailings.

I release ACST from any claims that may arise regarding the use of my name, image, or voice, including any claims of defamation, invasion of privacy, or infringement of moral rights, rights of publicity, or copyright. ACST is permitted, although not obligated, to include my name as a credit in connection with the image. I also waive the right to inspect or approve versions of my image used for publication or the written copy that may be used in connection with the images, relating to the project titled above.
